Revise estates and wills provisions for assisted reproduction and gestational agreements

To require an assisted reproduction facility to make available an optional declaration of intent form to establish an individual's consent to participate in assisted reproduction with a prospective birth mother, with the intent of being treated as the other parent of the child for purposes of estates and inheritance.
