Revise abortion complication reporting law

To require state public health officials to create a form that can be used by an individual who has actual knowledge of a woman who has sought treatment for a physical complication that is a result of an abortion, and which was not reported to the state by the abortion provider as required. The bill establishes procedures to record the information in a manner that does not violate patient privacy.
