Require pre-release mental health discharge plan for prisoners

To require the Department of Corrections to create a pre-release mental health discharge plan for each prisoner who is receiving mental health services or drugs before being released on parole, and require the Department of Health and Human Services to help in creating the required plans. The bill prescribes what such a plan must include.
