Revise managed care MMQAA tax details

To revise details of the “Michigan Medicaid Quality Assurance Assessment” tax on managed care organizations, limiting the use of the tax to paying for Medicaid, repealing it if the federal government no longer allows Medicaid matching funds based on state collections under the tax, and clarifying that entities previously subject to the tax remain so notwithstanding technical revisions in other statutes and regulations. This MMQAA is a bed tax on health care providers that is used to generate more federal Medicaid matching funds, resulting in a net gain for some but not all of the providers.
