Revise county Medicaid nursing home details

To extend until 2022 a freeze on “maintenance of effort” rates for long-term care services provided by county medical care facilities. Counties providing Medicaid nursing home services in county-owned facilities must reimburse the state according to a county "maintenance of effort" rate determined on an annual basis for each patient day. The freeze would otherwise end after 2017, and would result in higher payments to the state for Medicaid by some counties.
