Revise human artificial insemination disclosures

To allow a human artificial insemination service to provide a sperm donor with the option to allow the disclosure of his identity to the a child that issues from the sperm. The information could then only be released upon written request from the person who was conceived from the sperm, and the person would have to be age 18 or older when making the request.
