To require a physician, before prescribing medication to a minor, ordering a diagnostic test for a minor, or providing medical treatment to a minor, to first obtain written, informed consent from a parent, guardian, or person in loco parentis. The bill would require the Department of Community Health to devise a model policy and form for dealing with these issues.
