Revise government mental health services

To require that all community mental health services programs, state facilities, and licensed hospitals annually provide the Department of Community Health with summaries of and analysis on all known instances of death, serious injury, serious physical illness, incarceration, homelessness, and delay or interruption of education among mental health care recipients and applicants; require the state office of recipient rights to review and investigate as warranted; and require the information to be included in and annual report to the legislature.
